Sports Physicals at Community Clinic at Walgreens We are excited to announce that our Community Clinic at Walgreens sites will be providing sport physicals for student athletes from our partner schools for $30, starting on Monday, 7/7/2025 and ending on Sunday, 7/20/2025 - Mon-Fri 8:30 AM - 7:00 PM; Saturday and Sunday 8:30 AM – 5:00 PM (Last appointment is 20 minutes prior to close). The following is additional information regarding these $30 physicals: • $30 physicals will be at Community Clinic at Walgreens sites. o Urgent Care at MedCheck and Community Care Walgreens Clinics | Community Health Network (ecommunity.com) o Note: Offer NOT available at MedCheck locations • Community Clinic at Walgreens sites do not accept cash and payment is due at the time of the visit. • Please schedule an appointment either by phone or online (information in link above). Please come 10 minutes early to your appointment. o When Scheduling Online, please indicate Sports Physical and the School Name in the Visit Reason • The student-athletes must have the IHSAA form completed and signed by the parent/guardian including the past medical history (PMH) prior to the scheduled appointment. o If the patient has Medicaid, CHNw will need a non-cover letter. • Student-athletes will need to bring their glasses or wear their contacts to the appointment for the vision assessment. • If the student-athlete has not been cleared from a concussion by a physician, we will not be able to complete the sports physical.
